“Across the continuum of Elder Care services…” It sounds like something from a sci-fi movie intro, but the term “continuum” is getting used a lot in health care these days.

Here at Tabitha, we’re proud to say we were the first to offer Nebraska a complete continuum of Elder Care services. But what does that really mean?

A continuum of services offers care options at a variety of levels. For Tabitha, that means everything from home health care and senior living community options to hospice care and everything in between. At the center are professionals we call “continuum navigators” who work with families to determine where they fit on the continuum of services.

Navigators explore goals, care needs and discuss options with patients and their families. They make home visits, set up calls with family members and ensure all questions are answered. They’re the experts so you don’t have to be.

Ultimately, they create care plans that fit to the individual and as care needs change (as they frequently do as we age), the plan adjusts—perhaps adding new services or taking them away.

In my career, I’ve helped countless families along the aging journey and I’ve seen the looks of relief when family member and patients alike know they have an expert to give them answers, know they have  shoulder to lean on when the road gets rough and know they have a trusted organization to turn to when new health problems arise. That’s the difference of a health care continuum.
