GracePointe Assisted Living & Memory Care Suites Garden Square Assisted Living of Crete |

Tabitha Health Care Services is proud to celebrate the first anniversary of four of its senior living communities: GracePointe Assisted Living & Memory Care Suites; Martha House and Elizabeth House Green House® homes; and Journey House SNF for Hospice and Palliative Care.
Joyce Ebmeier, Tabitha’s senior vice president of strategic planning, led the development initiatives of each new community along with a dedicated team of architects, general contractors and Tabitha professionals,.
“Last year was significant in Tabitha’s history,” Ebmeier said. “As a result, our campus looks more beautiful than ever. But that’s not why we worked so hard to open these communities. From beginning to end, the projects were about serving the people of Tabitha—Elders, family members and staff. The buildings are vessels for living by Tabitha’s mission. They are tools for achieving the best in compassionate care.”
Tabitha extends heartfelt appreciation for the team members who contributed to the opening of each community and the care teams who provide daily, compassionate support for residents who have called Tabitha home for the past year.
GracePointe Assisted Living & Memory Care Suites is a 63-bed community designed for Elders who are not yet in need of skilled nursing care yet require assistance with the activities of daily living. In 2012, the community received a "Best of 50+ Housing" award from the National Association of Home Builders’ 50+ Housing Council for its innovative, retro-style design that invites Elders to relive precious memories of their early years.
Tabitha added the third and fourth Green House homes, Martha House and Elizabeth House, on its Lincoln campus in 2011. Green House homes step outside traditional long-term care and work to de-institutionalize residents’ lifestyles. Each house is home to 9-12 residents who are encouraged to live life on their own terms while receiving compassionate skilled nursing care. Green Houses promote the individuality and dignity of Elders no longer able to stay in their own homes.
Also opened last May, Journey House SNF for Hospice and Palliative Care is Lincoln’s first and only Medicare-certified skilled nursing hospice community. It provides loving and compassionate care in a home-like setting for individuals in need of skilled nursing during their end-of-life journey.
As a nonprofit organization serving 28 Nebraska counties, Tabitha’s love embraces a society where everyone is valued and empowered to live life to the fullest, with compassionate at-home support, innovative living communities, exceptional rehabilitation, health care and hospice services.
Tabitha, Your Answer for Elder Care, is proud to announce the promotion of Jeremy Hohlen to Vice President of Operations. In his new position, Hohlen will oversee operations in Tabitha’s home care programs, including medical, non-medical home support and hospice; as well as operations in Tabitha’s senior living communities ranging from assisted living, skilled rehabilitation nursing, to Tabitha’s Green House homes for long-term care.
“I am extremely blessed to have the opportunity to lead and work alongside an exceptionally talented, committed and knowledgeable group of team members,” said Hohlen. “Our teams are driven every day to ensure Elders receive the best care possible. I look forward to being part of Tabitha’s continued tradition of innovation and excellence.”
Hohlen joined Tabitha in 2008 as administrator of Tabitha Nursing & Rehabilitation Center in Lincoln and oversight of Tabitha Nursing Center in Crete. During the course of his leadership, both communities achieved and maintained the Centers for Medicare and Medicaid Services’ highest quality ranking – five out of five stars. This year, the communities were honored as “Best Nursing Homes in Nebraska” by U.S. News & World Report.
“Jeremy has consistently demonstrated courageous leadership in his varied roles at Tabitha. He shows commitment and compassion to our work together as well as all those who have the opportunity to work with him,” said Christie Hinrichs, Tabitha’s president & CEO.
Hohlen is an active member of the Nebraska Health Care Association, Nebraska Assisted Living Association and is Leading Age Nebraska board member. In his spare time, he coaches YMCA soccer and basketball, midget football and Lincoln Jr. Saltdogs Baseball.

Tabitha Health Care Services—celebrating 125 years—shows its commitment to Elder Care with the opening of GracePointe Assisted Living & Memory Care Suites and two new Green House homes. The openings will be celebrated at a public grand opening event Tuesday, April 26, 2:00-4 p.m. The event will be hosted at GracePointe, 4620 Randolph Street.
Opening May 2 is GracePointe Assisted Living & Memory Care Suites. This 63-bed facility is designed for Elders who are not yet in need of skilled nursing care, yet may like a bit of extra help around their home. Forty-eight residences will be dedicated to memory care. Charyl Lentz, life enrichment coordinator for GracePointe, will head Promote Life, an innovative lifestyle program for residents.
“Promote Life offers activities designed to maintain activity in the mind, body and soul while promoting health, independence and social interactions in a supportive environment,” said Lentz. Among a myriad of opportunities, residents can participate in spiritual groups, cooking clubs, the Memories in the Making painting group (an Alzheimer’s Association initiative) and Go4Life-Fitness program (provided by National Institute of Aging).
In addition to GracePointe, Tabitha will add two new Green House homes to its care continuum. Green Houses are a new choice for individuals requiring skilled nursing services. Tabitha Health Care Services is committed to be the answer for Elder Care in our community. They extend that commitment this spring with the opening of four new living communities for Elders: GracePointe Assisted Living & Memory Care Suites; two additional Green House® homes; and the Journey House for Hospice Care.
Opening May 2 is GracePointe Assisted Living & Memory Care Suites. This 63-bed living community is designed for Elders who are not yet in need of skilled nursing care, yet require assistance with the activities of daily living. The community features maintenance-free residences that provide around-the-clock personal care as well as activities focused on health and wellness. Forty-eight residences will be dedicated to memory care. Starr Carmichael, administrator at GracePointe, said the community includes Health Sense Technology, a quiet monitoring system offering extra safety measures for residents. “GracePointe suites are designed to provide meaningful and individualized care for Elders,” said Carmichael.
In addition to GracePointe, Tabitha will open two Green House homes to the two existing homes added in 2006 and 2010. Green Houses are a new choice for individuals requiring skilled nursing services. These communities take the institution out of the picture as they are designed with the home in mind. Joyce Ebmeier, vice president of strategic planning at Tabitha, said these 12-resident homes are unique from institutional living in that they are built to support and lift up individuals to live life on their own terms and schedules. “Green Houses foster a true partnership between caregivers and Elders,” said Ebmeier. “This relationship is focused on making each day fulfilling and meaningful.”
For over 30 years, Tabitha has led the community in providing compassionate hospice care. That commitment continues this spring with the opening of the Journey House for Hospice Care, Lincoln’s only Medicare certified hospice skilled nursing facility. The Journey House is a 12-resident home dedicated to care for Elders, their families and loved ones in this life’s final journey. The 8,100-sq.-ft. home offers private rooms designed with home in mind. Outside spaces and a chapel provide tranquility and comfort for families.

Tabitha hosted a reception for U.S. Senator Ben Nelson receiving the Chester A. Sterling Friend of Housing award for his support of affordable housing, June 3, 2010. Pat Doran, SVP, General Counsel & Corporate Secretary Federal Home Loan Bank System’s Affordable Housing Program (AHP) presented the award to the Senator as part of its 20th anniversary celebration.
“Tabitha has captured what seniors really seek in their life, which is a home-like setting, rather than a large institution or a big facility,” said Senator Nelson.
Senator Nelson has supported housing initiatives by voting for rural revitalization legislation that provides additional low-income housing tax credits for housing in rural disaster areas. Senator Nelson also supports legislation to provide needed liquidity through the FHLBank system to community banks, credit unions and thrifts.
Tabitha’s Green House has received $750,000 in Affordable Housing Projects (AHP) grants used to construct two green homes, which provide long-term care in a home setting for 24 older adults. Tabitha’s first Green House home (the nation’s second) opened in May 2006. The second Green House opens July 7, 2010. Two additional homes have broken ground and will open in the summer of 2010.
The Friend of Housing award is named after Chester A. Sterling, FHLBank Topeka’s first president. Under his leadership, FHLBank Topeka started with a few million dollars in capital and grew to assets of $96.6 million and 235 member banks by the end of his 24-year career.

Tabitha will accept this award at the LSA Awards Banquet scheduled for St. Louis on April 24th.
The Green House Project at Tabitha provides a new option for nursing home care by de-institutionalizing the lives of Elders requiring long-term care.
