Referrals, Admissions & Questions

Tabitha Case Management services are available 24 hours day, seven days a week. Case Management coordinates admissions to Tabitha Nursing & Rehabilitation Center.

The team also coordinates referrals for Home Health Care, Hospice, Home Care Specialties (private-pay home care), Outpatient Rehabilitation Therapy, Meals on Wheels, Adult Day Services, Telehealth daily vitals monitoring and VoiceCare emergency response system.

Our Case Managers work closely with hospital discharge planners and professionals at other nursing and assisted living facilities to ensure smooth transition and continuity of care between facilities. We are available to answer questions about any of Tabitha's services, financial/payment concerns, placement needs, and other community resources. We can also provide home visits to discuss placement or home health needs.
