Referrals, Admissions & ResourcesTabitha Case Management services are available 24 hours day, seven days a week. Case Management coordinates admissions to Tabitha Nursing & Rehabilitation Center, as well as referrals for Home Health Care, Hospice, Home Care Specialties (private pay home care), Outpatient Rehabilitation Therapy, Meals on Wheels and Adult Day Services. Our Case Managers work closely with hospital discharge planners and professionals at other nursing and assisted living facilities to ensure smooth transition and continuity of care between facilities. We are available to answer questions about any of Tabitha's services, financial/payment concerns, placement needs, and other community resources. We can also provide home visits to discuss placement or home health needs. |
