Vivian’s Story

Vivian spent the prime of her life as a nurse, saving lives and teaching others. During those years, she couldn’t imagine one day needing help of her own. But that day came and after a stay at the hospital for congestive heart failure and respiratory issues, Vivian and her family found themselves needing help.

Tabitha Home Health Care began supporting Vivian with nursing care following the hospital stay and has continued supporting her several years on with caregiver visits, companionship and clinical services. A team of home health aides and nursing staff work together with Vivian’s family and physician to manage her illness, keeping her independent and out of the hospital.

Vivian’s Tabitha Home Health Care nurse explains that the partnership between a trusting client, supportive loved ones, home health provider and personal physician is extremely important in maintaining Vivian’s health, and it is this team effort that has helped Vivian stay at home.

Every care plan developed by Tabitha Home Health Care begins with a home safety evaluation. During this process the living environment is assessed for risk factors from mobility/fall hazards to nutritional needs, hygiene processes, emergency care access and more. Once risks are mitigated or eliminated, home health care staff may provide ongoing education and training to the client and their family members or other loved ones in order to maintain safe and secure surroundings where the client can continue to progress and thrive. In-Home Technology

VoiceCare emergency response supports independent living with a secure, reliable and live voice-to-voice system. Clients simply wear a personal activator button that, when pressed, will contact designated responders such as family, neighbors or emergency response personnel.

Telemonitoring offers a convenient service that regularly collects and transmits vital signs (blood pressure, weight, oxygen level and pulse) through a secure, cellular modem as frequently as our clients and physicians desire. When vital signs are taken daily, irregularities can be caught before they become full-fledged problems. Daily monitoring helps eliminate the two- or three-day gaps of information that can occur between home health visits.  Seniors also become more aware of their condition and understand the effects diet, activity and other factors have on their health.

Technology options from Tabitha offer a simple but important way to keep tabs on health between regular physician and/or home health visits.
